Terms of Behavior Assessment & Modification Plan
By signing below you grant permission to evaluate and work with your cat for behavioral complaints, concerns and problems.
Completion of the Initial Consultation and Behavior Assessment History form is required to be completed before we can evaluate your cat. The information you provide on this form and during the appointment is considered confidential. Please keep a copy of the history form for your own records.
If you have had any behavioral evaluations or physical/laboratory exams done elsewhere, it would be helpful if we had a copy before your appointment. Please email to AllAboutPurrs@cs.com.
You may also be asked to provide videos and/or photos of your cat. We request these because it is the best way to see the behaviors that concern you within the context of everyday life. The video should:
- Show the behavior(s) about which you are concerned, except for any injurious behavior
- Give a brief tour of the cat’s environment (house/apartment/patio/yard)
- Show any other relevant facets of the cat’s life (e.g., where your cat sleeps, sits, eats, drinks, walks, plays, interaction with other animals, etc.)
If aggression with injury is one of the complaints, please do NOT provide a video of the cat biting someone, and do not put the cat in a situation where any aggression may be provoked. If aggression is a concern, other behaviors will be indicative of it.
Videos and/or photos may also be taken during the appointment. Any videos/photos used to evaluate the cat become part of All About Purrs’ client record and may be used anonymously in all modes of teaching (including teaching staff or other clients) and/or research.
By completing these forms and submitting the initial consultation form, you give permission for us to evaluate, assess and formulate a behavior modification plan, and to work with your cat. You will receive a written copy of the 2-page behavior modification plan, and we encourage you to comply with our recommendations and to ask questions at any time.
The evaluation, assessment, and modification plan do not represent a guarantee of successful behavior modification. All cats are unique, just like humans, with varied life experience, genetics and backgrounds. Few behavioral problems are truly cured, and responsible environment management is a factor for every cat.
WAIVER, ASSUMPTION OF RISK AND AGREEMENT TO HOLD HARMLESS
I understand that the process of behavior change is not without risk to myself, or members of my family, because animals may be the cause of injury even when handled with the greatest amount of care.
I hereby waive and release Lisa Emrich and All About Purrs, it’s employees, owners and agents from any and all liability of any nature for injury or damage which I, my property or my pet may suffer, including specifically, but without limitation, any injury or damage resulting from the action of any pet, and I expressly assume the risk of any such damage or injury while attending any behavioral sessions or other functions of Lisa Emrich and All About Purrs on my property, or while on the property or the surrounding area thereto.
In consideration of and as inducement to the acceptance of my application for a behavioral appointment, I hereby agree to indemnify and hold harmless Lisa Emrich and All ABout Purrs, its employees, owners and agents from any and all claims, or claims by any member of my family or any other person accompanying me to any training session or function of Lisa Emrich and All About Purrs on my property, or while on the grounds or surrounding area thereto as a result of any action by any pet, including my own.
